Thank you for your interest in requesting an vote by mail ballot for the upcoming election. You must be a registered voter to submit an application.

If you don’t have an Illinois Driver’s License or ID Card and the Last 4 digits of your Social Security Number, you will need to fill out and sign a paper Vote by Mail Application. Once complete, send the application to the Champaign County Clerk’s Office by email, mail, or in-person.

Instructions:

  • You must have an Illinois Driver’s License or Identification Card and/or Last 4 digits of Social Security Number to complete the Online Vote By Mail Ballot Request.
  • If you do not have one of these ID numbers, you will need to download a paper application and mail it to your County Board of Elections. Otherwise, you may visit your local county elections office.
